NEW PATIENTS

Initial Appointment

Your first visit includes a consultation to review your diagnosis and treatment options. Sometimes, surgery can be done the same day, but complex cases may require additional appointments.

Please bring the following information at the time of your consultation:

  • Your surgical referral slip and any x-rays if applicable.
  • A list of medications you are presently taking.
  • New Patient Registration form with insurance information – please complete online, print, and sign if applicable.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

Important: Patients under 18 must be accompanied by a parent or guardian.

Patient Forms:

At Cliffs Oral Surgery, your health and comfort are our priority. To help us provide the best care and keep appointments on schedule, please review and complete the necessary forms before your visit. You can download, print, and fill them out in advance to save time and reduce waiting.

For IV anesthesia surgery:

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.  Please fast (no food or drink) for 6 hours before surgery and arrange for an adult to drive you home.

Medical Alerts:

Inform us of any medical conditions (e.g., diabetes, high blood pressure, artificial valves/joints) or medications (e.g., heart meds, aspirin, blood thinners) before surgery.

X-Rays:

If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.
